Travel Details & Itinerary
1.1

This Parental Authorization for Minor Travel ("Authorization") is executed by the undersigned parent or legal guardian (the "Consenting Parent/Guardian") for the minor child identified on the signature page (the "Child"). The Consenting Parent/Guardian represents that he or she holds lawful legal custody of the Child and has full legal authority to authorize the travel described herein. The authorized travel shall commence on the departure date and conclude on the return date specified on the signature page, during which time the Child is authorized to travel to the destination(s) identified in Exhibit A.

1.2

The complete travel itinerary, including all destinations, transportation modes, carrier names and flight numbers (if applicable), accommodation names and addresses, and anticipated route, is set forth in Exhibit A. Any material deviation from the approved itinerary, including changes in destination, extension of travel dates, or substitution of transportation methods, requires the advance written consent of the Consenting Parent/Guardian. The Consenting Parent/Guardian shall be notified promptly of any unplanned deviation from the itinerary caused by emergency circumstances.

1.3

If the authorized travel involves international destinations, the Consenting Parent/Guardian acknowledges that additional documentation may be required by foreign governments and border agencies, including apostilled or notarized consent forms, translated copies of this Authorization, and proof of the parental relationship. The Consenting Parent/Guardian shall prepare and provide such additional documentation as may be required to ensure the Child's lawful entry into and departure from each destination country, consistent with the destination country's applicable immigration and entry requirements.

Accompanying Adult Authorization
2.1

The Consenting Parent/Guardian authorizes the Child to travel under the supervision and care of the accompanying adult identified on the signature page (the "Accompanying Adult"), who is described by full legal name, date of birth, government-issued identification type and number, relationship to the Child, and contact telephone number. The Consenting Parent/Guardian represents that he or she has verified the identity and trustworthiness of the Accompanying Adult and consents to the Accompanying Adult serving as the Child's temporary guardian for all purposes during the travel period.

2.2

The Accompanying Adult shall be responsible for the supervision, care, safety, and welfare of the Child at all times during the travel period. The Accompanying Adult shall not delegate supervisory responsibility to any third party without the prior written consent of the Consenting Parent/Guardian, except in genuine emergencies. The Accompanying Adult shall comply with all applicable laws of the jurisdictions through which the Child will travel and shall ensure that the Child's passport and travel documents remain secure throughout the travel period.

Medical Emergency Authorization During Travel
3.1

The Consenting Parent/Guardian hereby authorizes the Accompanying Adult to consent to any and all medical, surgical, dental, or emergency treatment deemed necessary by a licensed physician, surgeon, dentist, emergency medical technician, or other qualified healthcare professional for the health and welfare of the Child during the travel period, in the event that the Consenting Parent/Guardian cannot be reached in a timely manner. This medical authorization includes the administration of anesthesia, blood transfusions, intravenous medications, and such surgical procedures as the treating physician deems necessary in a life-threatening emergency.

3.2

Known allergies, current medications, significant medical conditions, and health insurance information for the Child are set forth in Exhibit B. The Accompanying Adult shall carry a current copy of Exhibit B at all times during the travel period and shall present it to any healthcare provider rendering treatment to the Child. The Accompanying Adult shall notify the Consenting Parent/Guardian of any medical treatment provided to the Child during the travel period within twenty-four (24) hours of such treatment, or as soon as communication is reasonably possible given the circumstances.

Notarization & Authentication
4.1

This Authorization shall be executed before a notary public or other officer authorized to administer oaths in the jurisdiction in which the Consenting Parent/Guardian signs. The notarization attests to the identity of the Consenting Parent/Guardian and the voluntariness of the signature. A notarized original or certified copy of this Authorization should accompany the Child throughout the travel period and should be presented to any border, customs, immigration, or transportation authority requesting documentation of the Child's authorization to travel.

4.2

For international travel, the Consenting Parent/Guardian should obtain an apostille pursuant to the Hague Convention Abolishing the Requirement of Legalisation for Foreign Public Documents (the "Apostille Convention") if the destination country is a signatory thereto. If the destination country is not a signatory to the Apostille Convention, the Consenting Parent/Guardian should seek authentication or legalization through the appropriate diplomatic or consular channels. The Consenting Parent/Guardian is advised to verify the specific documentation requirements of each destination country prior to the departure date.
